[MPEG-OTSPEC] Specification and "implementors' notes"

There is a delicate balance of things we can do in a lifetime vs. the impact and the outcome of things we chose to do. Considering that our time resources [and lifetime itself] are limited, we should choose wisely. ;)
When it comes to comments, examples, and notes as part of the spec, I think that having them as a resource to facilitate the understanding of the spec is useful, even if we end up authoring another implementation guidelines document.

It does. The whole spec needs to be edited and rewritten to separate what the actual specification is versus what the Microsoft editor's understanding of what the specification is is.
