[MPEG-OTSPEC] Near-future meeting management (and some background for “the meeting”)

Thanks Vlad! Indeed referring informally to a 'meeting chair' could invite
confusion with your formal role as ISO Ad Hoc Group Chair, so thanks for
pointing this out - I think using the terms 'host' and 'facilitator' is
much better :)
