<calt> should be on by default, but as defined it is clearly intended 
that the user might turn it off, as is possible in e.g. InDesign. In 
some typefaces, turning off <calt> alters the appearance of the text but 
does not break it (consider, for example, a Latin script type such as 
Adobe's Bickham Pro). I can also imagine designs in which some 
contextual shaping would be considered optional and some required; 
hence, two features seem desirable.

PS. If we were starting OpenType from scratch, I would have suggested a 
lookup level toggle for optional/required.
